PERIODONTAL POSTOPERATIVE INSTRUCTIONS
1. CARE OF THE MOUTH : You should avoid eating hard food or drinking of warm fluids for at least 24 hours. Do not smoke until your next visit. Do not rub the face in the area of surgery, and if you lie down, do not lie with any pressure against the surgical area. No carbonated beverages or alcohol for at least a week after the surgical procedure. No spitting, using a straw or rinsing for the first 24 hours.
2. IN CASE OF SURGICAL DRESSING : Occasionally small pieces of the dressing break off in the days following surgery. If the uncovered area is small and feels comfortable, it is perfectly safe to wait until your next appointment. However, if a large piece should break off, or if the dressing should feel loose, call the office as soon as possible. Be cautious of anything too hot, spicy, or hard to chew.
3. BRUSHING : On the day of surgery teeth should not be brushed. On the day following the surgery, brushing without toothpaste and flossing of teeth (other than the surgical area), and rinsing of mouth should be started. Frequent gentle rinsing of the mouth with warm water or prescribed mouthwash will aid the healing process and keep the mouth clean. Toothpaste can normally be resumed following the first follow up appointment.
4. PAIN : Following all types of surgery a certain amount of discomfort or pain can be expected. Usually this is slight and can be controlled by taking the prescribed medications. Use the ice pack for an hour after the surgery only. Place the ice pack in the surgical area in five minute intervals for one hour. Avoid clenching or gritting teeth after periodontal surgery. Pressure on the teeth will cause pain.
5. SWELLING : A slight amount of swelling and discomfort is not unusual following some procedures and usually goes away within two or three days. In most instances, these problems can be prevented or controlled if the instructions in the previous paragraphs are followed. If swelling begins, it can usually be controlled by use of a wet warm compress applied to the outside of the face in the area every 15 minutes for an hour. If swelling persists for more than 24 hours, contact the office or phone the doctor on his cell phone if it is after office hours.
6. BLEEDING : Do not rinse for the first 12 hours. Slow oozing may occur for several hours. One drop of blood will discolor saliva to a light red color. If bleeding is heavy:
- Apply dry gauze or a tea bag moistened in warm water on the bleeding area and hold with continuous pressure for 30 minutes avoiding spitting. Repeat if necessary.
- Sit up in bed or prop head up with 2 or 3 pillows.
- If bleeding continues call my office 602-956-0650.
7. DIET : Eating should be avoided until numbness is gone following surgery. It is best to restrict your diet to fluids and very soft foods until after your first follow up appointment. Milk, eggs, custards, ice cream, ground meat, milkshakes and liquid diet foods are all very nutritious and require little or no chewing. Chewing should be done on the side opposite the surgical area.
WARNING
DO NOT DRINK ALCOHOLIC BEVERAGES, DRIVE OR OPERATE HEAVY MACHINERY WHILE TAKING NARCOTICS SUCH AS VICODIN OR PERCOCET.
ALWAYS TAKE MEDICATIONS WITH A FULL GLASS OF MILK OR FOOD TO AVOID STOMACH UPSET.
FEMALE PATIENTS TAKING ANTIBIOTICS AND BIRTH CONTROL MEDICATIONS SHOULD USE ANOTHER METHOD OF BIRTH CONTROL AND CHECK WITH YOUR PHYSICIAN ABOUT WHEN THE CONTRACEPTIVE METHOD YOU ARE USING WILL BE EFFECTIVE AGAIN.
Instructions after Sinus Lift Procedure
You have had a Sinus Lift Augmentation procedure in your upper jaw. This procedure regains lost bone height in the area of your first and second molars and occasionally the second premolar. It is an important procedure as it allows implant placement in an area that could not be implanted otherwise because of insufficient bone height due to an enlarged sinus.
The bone that has been grafted is most commonly a combination of freeze-dried bone and allograft.
Do not under any circumstances blow your nose for the next four weeks. This may be longer if indicated. You may sniff all you like but no blowing.
1. CARE OF THE MOUTH : You should avoid eating hard food or drinking warm fluids for at least 24 hours.
2. ANTIBIOTICS : Be sure to take the prescribed antibiotics as directed to help prevent infection.
3. ORAL HYGIENE : Do not rinse or spit on the day of the surgery. This tends to disturb the blood clot, open the wound and can prolong bleeding and slow healing. You should not have a significant amount of blood in your mouth. Saliva can be swallowed, even if slightly blood tinged.
Keeping your mouth clean after surgery is essential to reduce the risk of infection. Use the prescribed mouthwash twice a day starting the day after the surgery. Do not brush the teeth in the area of the surgery for 48 hours. When brushing, be gentle. When spitting, also be gentle.
4. PAIN : Following all types of surgery a certain amount of discomfort or pain can be expected. Usually this is slight and can be controlled by taking the prescribed medications. Use the ice pack for one hour after the surgery only. Place the ice pack in the surgical area in five minute intervals for one hour. Avoid clenching or gritting teeth after periodontal surgery. Pressure on the teeth will cause pain.
5. SMOKING : Do not smoke for at least two weeks after surgery, if at all. As discussed at your consultation, smoking dramatically increases the risk of bone graft and sinus augmentation failure.
6. SWELLING: A slight amount of swelling and discomfort is not unusual following this procedure and usually goes away within two or three days. In most instances, these problems can be prevented or controlled if the instructions in the previous paragraphs are followed. If swelling persists for more than 24 hours, contact the office or phone the doctor on his cell phone if it is after office hours.
7. BLEEDING : Do not rinse for the first 12 hours. Slow oozing may occur for several hours. One drop of blood will discolor saliva to a light red color.
9. Wearing your Prosthesis or Nightguards : Do not rinse for the first 12 hours. Slow oozing may occur for several hours. One drop of blood will discolor saliva to a light red color.
10. Post Operative Problems or Complications: As with any procedure, unexpected post-operative healing can occur. If you notice the unexpected flow of air or liquids between your mouth and nose, please let us know immediately.
If you are aware of several small particles of graft material being discharged from your nose, let us know.
If there is an increase in swelling in your mouth, cheek or under your eye after three days, let us know.
It is our desire that your recovery be as smooth and pleasant as possible. Following these instructions will assist you, but if you have any questions about your progress, please call our office 602-956-0650.

Extraction postop instructions
1. BLEEDING : Normal oozing may occur for up to 24 hours following surgery. Firm biting on a gauze pad placed over your tooth socket controls it. Change pack every 20 minutes for the first hour and a half. Gauze packs should by lightly dampened with water. If bleeding does not lessen, please call the office immediately. Our office telephone: (602) 956-0650.
2. SWELLING : Swelling and sometimes bruising is common after surgery. The maximum swelling, pain and jaw stiffness normally occurs two to three days after the surgery. Apply cold packs in five minute intervals for one hour the first day of surgery. This will keep swelling to a minimum. It will help to keep your head elevated. Ibuprofen or Motrin as prescribed will help with the swelling.
3. PAIN : The most discomfort is in the first 6 to 8 hours after surgery. Take pain medication before the numbness wears off with plenty of water or a light food, and then as needed and prescribed after that. If itching or rash develops, stop taking medication immediately and contact the office. If dull ache along with an earache begins on the 3rd or 4th day after surgery, please contact the office, as this may be an indication of a dry socket, which can be treated with the proper medication and relieves discomfort quickly.
4. MUSCLE SORENESS : Difficulty in opening the jaw is common after third molar extractions. Moist heat may be applied 36 hours after extractions if soreness continues.
5. MOUTH CARE : No rinsing, brushing, spitting or smoking for 24 hours following surgery. The day after surgery rinse with a glass of warm water mixed with one teaspoonful of salt three times a day or use the prescribed oral rinse for a week. After 24 hours, brush and floss as usual avoiding the surgical sites.
6. DIET : It is important to maintain good nutrition following surgery. A cool soft diet the day of surgery is advisable. No carbonated beverages, smoking, spitting or alcohol for the first 48 hours. Do not drink through a straw. Eat whatever is comfortable the day after surgery, and drink as many fluids as possible for the next 5 days.
7. INFECTION : If swelling and discomfort continues or increases after the third day following surgery, or if you experience a foul taste, fever or difficultly swallowing, contact our office immediately.
It is not uncommon for small chips of bone to work their way through the tissue following extraction. In some cases, it is necessary for the doctor to remove them. Please call our office if you need us to check the area.

2. ACTIVITY : Avoid strenuous physical activity, i.e. weight lifting, running, swimming, etc. for 48 to 52 hours following surgery. Get plenty of rest and resume normal activity as you are comfortable. Complete rest for the first 6-12 hours significantly reduces discomfort, decreases swelling and speeds healing.
3. BRUSHING : If you had a Connective Tissue Graft do not brush the grafted area until we evaluate healing at your first following visit. Start the prescribed mouthwash 1 to 2 days after the surgical procedure.
- Apply dry gauze or a tea bag (Lipton) moistened in warm water on the bleeding area and hold with continuous pressure for 20 minutes avoiding spitting. Repeat if necessary. The caffeine and tannic acid in the tea bag will help to clot and stop bleeding. Never apply pressure in the area were the connective tissue graft was placed.
8. SUTURES : Do not remove your own sutures as they can compromise healing. Sutures used in gingival augmentation procedure most of the time will reabsorb in two to three weeks.
